Step 2: Obtain Your Driving Record
Please purchase your driver record from the Colorado DMV. Click on Driver ID services, Records, Request Driving Record, and make sure to choose the option to email the record to yourself. After obtaining your driving record, email it to us at application@goldrushtransportation.com.
Step 3: Obtain a Medical Examination Report
You need to obtain a medical examination report. This report must be filled out by one of the following: a doctor of medicine or osteopathy, a physician assistant, nurse practitioner, or clinical nurse specialist working under the direct supervision of a physician.
After completion, please email a copy of the medical card to us at application@goldrushtransportation.com.
